All invoices for clinicals records are sent via Square. Payment is required prior to our office releasing documents.

To request a medical legal report, please fax your request, with patient authorization to release information to 604-563-5604. Kindly note, we are an extremely busy office and may not be able to accommodate your request. 

WORKSAFE BC

We do not expedite appointments or surgeries for Worksafe BC. All patients are treated according to urgency. If you are requesting clinical copies from our office, please fax your request, along with patient authorization to release records, to 604-563-5604. We do require pre-payment to release records.
