PATIENT INFO

**Due to the Covid-19 pandemic, we are conducting all appointments via telephone until further notice.**

WHAT TO HAVE READY FOR YOUR APPOINTMENT:

--> List of current medications

--> Valid personal health number (PHN)

--> CDs of any private imaging that you have had done **this must be sent to our office PRIOR to your appointment.

MSP COVERAGE:

If you do not have valid MSP coverage, you will be responsible for the cost of your appointment. Please contact our office via the contact tab if you need more information in this regard.

NO-SHOWS/CANCELLATIONS:

Our office has a 24 hour cancellation policy. If you do not cancel and appointment with 24 hours notice, minimum, you will have a fee applied to your file that must be paid prior to rebooking.  If you miss a confirmed appointment and do not notify our office with at least 24 hours notice that you will not be attending, you will be responsible for a no-show fee prior to re-booking. 

INSURANCE/DISABILITY FORMS:

Please note, we do not regularly fill out insurance forms or disability forms. Please direct this request to your family doctor. 

OFFICE CONDUCT:

We have strict expectations regarding patient conduct when communicating with our office either in person, via email or by telephone. We have a zero-tolerance policy for any type of rude or abusive behaviour towards our staff or our doctors, and patients who are deemed to be acting in this manner will be discharged from our practice immediately.
